Cordaan zoekt Begeleider Dagbesteding voor De Boei op De Werf in Amsterdam Noord. Iets tussen 16 en 24 uur per week met regelmatige flexibiliteit.
Je coördineert zorg, werkt aan het verbeteren van de dagbesteding volgens Triple-C en stelt zorgplannen op en evalueert deze met de trajectcoördinator. Je hebt een mbo-4 zorgdiploma, bent communicatief en empathisch, werkt graag in team, en hebt bij voorkeur ervaring met mensen met een verstandelijke beperking.
