GGZ in Geest zoekt een tijdelijke casemanager voor één van onze FACT-teams, voor een periode van zes maanden.
Als casemanager bied je cliënten structuur, begeleiding en continuïteit van zorg, voer je intakegesprekken en huisbezoeken uit in samenwerking met een multidisciplinair team.
Je werkt met ouderen met EPA; herstelondersteunende zorg en netwerken in de wijk staan centraal. Rijbewijs vereist en je werkt zelfstandig binnen een collegiale sfeer.
