SIGRA in Amsterdam zoekt een coördinerend verzorgende IG die de verbindende schakel is in de wijk. Je werkt zelfstandig en biedt deskundige zorg, waarbij je nauw samenwerkt met cliënten en zorgprofessionals.
Je onderhoudt contact met huisartsen en andere ketenpartners, stelt zorgplannen op en signaleert veranderingen in gezondheid. Een vast contract voor 20 of 24 uur per week met aantrekkelijke arbeidsvoorwaarden, waaronder vakantiegeld en pensioenregeling via PFZW.
