BovenIJ zoekt een transferverpleegkundige die zelfstandig patiënten screent voor nazorg en het proces van door- en uitstroom coördineert. Je adviseert en ondersteunt patiënten en naasten bij (complexe) nazorg, intra- en extramuraal, en werkt in multidisciplinair overleg aan optimale zorg.
Je vormt de schakel tussen het BovenIJ en vervolgstappen, met nauwe contacten met zorginstellingen in de regio. Het team van 5 transferverpleegkundigen valt onder de manager ICM.
