In deze rol ben je verantwoordelijk voor de begeleidingstrajecten van bewoners, waarbij je doelen en acties helder vertaalt en de voortgang bewaakt.
Je werkt nauw samen met collega's en bewoners om een professionele en ondersteunende omgeving te creëren. Wij bieden een betekenisvolle functie in de gehandicaptenzorg met mogelijkheden voor professionele ontwikkeling.
