SIGRA in Amsterdam zoekt een Casemanager voor het FACT-team om cliënten te begeleiden in hun zorg. Als casemanager bied je structuur en aandacht, werk je binnen een multidisciplinair team en ondersteun je cliënten in hun herstelproces, zowel thuis als in de gemeenschap.
