Ons Tweede Thuis zoekt een begeleider op locatie De Kwakel die met geduld, humor en lef samen met cliënten werkt aan een zo zelfstandig mogelijk leven. Je draagt bij aan een veilige, betrokken leefomgeving waarin elke cliënt gezien en gehoord wordt.
Je ondersteunt het dagritme, help bij het ontdekken van beperkingen en kwaliteiten, en organiseert evaluaties en ontwikkelplannen. Daarnaast coördineer je (medische) zorg waar nodig en werk je samen met verwanten en collega’s aan een positieve impact.
