SIGRA zoekt een Jeugdmaatschappelijk werker FACT Jeugd Noord voor Amsterdam Centrum en Noord. Je werkt als primair behandelaar en casemanager binnen het FACT-Jeugd-team en begeleidt jongeren van 14 tot 24 jaar met ernstige psychiatrische problematiek.
Je organiseert behandeltrajecten, voert intakegesprekken en werkt nauw samen met ketenpartners. Een flexibele, zelfstandige aanpak en ervaring in ambulant FACT-vernieuwing zijn gewenst.
