SIGRA zoekt een Persoonlijk Begeleider voor De OID in Wognum. Je werkt intensief met collega’s en behandelaren en neemt regie over de begeleiding van cliënten met LVB en complexe problematiek.
Je bent het vaste aanspreekpunt voor meerdere cliënten, stuurt op opstellen en evaluaties van ondersteuningsplannen en onderhoudt contacten met verwanten en professionals. Je draagt bij aan een veilige, voorspelbare leefomgeving en de ontwikkeling van cliënten.
