GGZ in Geest is op zoek naar een casemanager voor de FACT-teams in Amsterdam, Amstelveen, Haarlem en Hoofddorp. Je levert zorg op maat aan volwassenen en ouderen met EPAs en werkt intensief samen met psychiaters, verpleegkundigen en andere professionals.
Je begeleidt cliënten in hun herstel, voert intakegesprekken en signaleert ontwikkelingen in hun gezondheid en welzijn. Je werkt nauw samen met het netwerk van de cliënt en streeft naar integrale zorg in de eigen omgeving.
