Stichting Philadelphia Zorg zoekt een Begeleider LVB & Community voor Amsterdam-Noord. Je begeleidt bewoners bij zelfstandig wonen, stimuleert verbindingen in de wijk en coördineert activiteiten in de buurtkamer.
Je begeleidt bij dagstructuur, huishouden en sociale contacten, en pakt ook complexere thema's aan zoals psychische problematiek of verslaving. Initiatief nemen en samenwerking met buurtorganisaties zijn sleutelpunten.
