Cordaan zoekt een persoonlijk begeleider voor Nieuw Sloten (Amsterdam Nieuw-West) die (jong) volwassenen met een LVB+ en gedragsproblematiek begeleidt. Je draagt zorg voor overzicht, structuur en samenwerking met familie en professionals, en werkt aan zelfstandig wonen en maatschappelijke participatie.
Het dienstverband betreft 32 uur per week met uitzicht op een vast dienstverband na positief functioneren. Salaris conform FWG 45, met aanvullende toeslagen en reiskostenvergoeding.
