Je werkt vanuit de mogelijkheden van de bewoner, betrekt het sociale netwerk en bevordert zelf- en samenredzaamheid, participatie en voorkomen van sociaal isolement.
Je ervaring met Wmo en doelgroepen als LVB, NAH en GGZ-problematiek wordt verwacht; je besluit zelfstandig bij passende ondersteuning.
