Stichting GGZ in Geest zoekt een trajectbegeleider voor een FACT-team in Amsterdam. Je begeleidt cliënten bij het vinden van passende dagbesteding en werkt nauw samen met een multidisciplinair team van specialisten en begeleiders.
Je bezoekt cliënten thuis of op een wijklocatie, signaleert wat er speelt en stelt samen met hen een plan van aanpak op om doelen te bereiken. De functie is vast en valt onder FWG 50 cao GGZ.
