• Master's Degree in Nursing from an accredited Nurse Practitioner program

• Active New York NP license (unrestricted)

• DEA registration for prescribing authority

• Eligible for Medicare/Medicaid billing

• 3+ years of clinical experience , preferably in geriatrics or long-term care

• Reliable transportation to travel between assigned communities

• Compassionate bedside manner, teamwork, and a great sense of humor

• Conduct comprehensive health assessments , physical exams, and medication reviews

• Develop personalized care plans in collaboration with the interdisciplinary team

• Manage chronic and acute conditions within your NP scope

• Order and interpret lab results and diagnostic tests

• Prescribe medications responsibly

• Educate patients and families on preventive care, wellness, and independence
