Primary Care Nurse Practitioner (NP)

We are seeking a skilled and compassionate Primary Care Nurse Practitioner to join our healthcare team in California. The ideal candidate will provide comprehensive primary care services to patients of all ages, ensuring high-quality and patient-centered care. This role requires excellent clinical skills, strong communication abilities, and a commitment to improving patient health outcomes.

• Conduct thorough physical examinations and health assessments.
• Diagnose and treat acute and chronic illnesses.
• Develop and implement individualized treatment plans.
• Prescribe medications and therapies as needed.
• Educate patients and their families on health maintenance and disease prevention.
• Collaborate with physicians and other healthcare professionals to ensure coordinated care.
• Maintain accurate and timely documentation of patient encounters.
• Participate in quality improvement initiatives and continuing education.

• Master's or Doctoral degree in Nursing from an accredited program.
• Valid Nurse Practitioner license in the state of California.
• Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP).
• Experience in primary care settings preferred.
• Strong clinical and diagnostic skills.
• Excellent interpersonal and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
