Geriatric Nurse Practitioner, Healthcare Nursing
A Nurse Practitioner position is opening at a Stockton, CA area PACE center to join an interdisciplinary care team in a center-based outpatient setting. You will provide primary care to frail, elderly participants and work with the supervising physician and IDT to develop care plans.
