Responsibilities
  • Collect and analyze patient data, identify expected outcomes, and develop a plan of care with interventions.
  • Collaborate with the specialty healthcare team and other disciplines to evaluate patient progress toward outcomes.
  • Coordinate care for a defined population using a systematic, evidence-based approach and maintain patient tracking and reporting programs to identify gaps in care.
  • Contribute to organizational improvement through participation in unit-based councils and process improvement activities.
  • Use nursing knowledge to assess, plan, implement, evaluate, and communicate patient care, working with the healthcare team.
  • Engage in ongoing quality assessment and improvement of nursing practice, aligning with professional standards and regulations.
  • Adhere to Standards of Practice for Case Management and the ANA Code of Ethics; serve as a consultant on specialty issues as needed.
  • Deliver ethical, patient-centered care that respects diversity and preserves patient autonomy, dignity, and rights.
  • Plan, coordinate, and communicate with patients, families, and payers to secure appropriate services and facilitate access to care.
  • Collaborate with external facilities for complex subspecialty needs and triage patients via telephone, email, and in-person encounters.
  • Provide extensive patient education on complex issues and disease processes; deliver timely and competent nursing care consistent with standards.
  • Perform program-specific nursing assessments, interventions, and evaluations, including specialized infusions and protocols as applicable.
  • Share specialty information with colleagues to enhance nursing quality of care.
  • Perform other duties as assigned.
Qualifications
  • Associates degree in Nursing (minimum).
  • Current registered nurse license in the State of Alaska;
    Basic Life Support (BLS) certification.
  • Minimum five years of professional health education, health-related research, or health program development/implementation/evaluation experience (non-supervisory); equivalent combinations may be substituted.
  • Preferred:
    Bachelor’s Degree in Nursing.
  • Preferred:
    Experience in the Alaska Tribal Health System.
  • Preferred:
    National Case Management certification through ANCC or other specialty certification.
Knowledge,

Skills and Abilities
  • Knowledge of nursing science, terminology, anatomy, physiology, disease concepts, evidence-based practice, and hospital operations.
  • Strong critical thinking, problem solving, and communication skills; ability to assess and prioritize multiple requests.
  • Proficiency in collecting patient data, analyzing findings, and using information technology tools.
  • Understanding of Alaskan culture and shared governance in nursing practice; ability to practice ethically and nondiscriminatory care.
