Overview
Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to join our team. You will deliver person-centered care in the home environment, assess patient and family needs, and work with the Interdisciplinary Group (IDG) to develop and direct individualized plans of care. This is a full-time, 40-hour per week position with day shifts and an on-call requirement.
Schedule: 5x 8-hour shifts, with on-call duties. This is a collective bargaining position.
Position SummaryJoin a team where compassionate care meets clinical excellence. You’ll assess patient and family needs, provide professional nursing care grounded in the full nursing process, and collaborate with the IDG to develop and direct individualized plans of care. You’ll support patients and families with empathy and a commitment to helping patients live with dignity and comfort during end-of-life care.
The Hospice nurse builds from the resources of the IDG to plan and direct services to meet the needs of individuals and families within their home environments. May provide patient care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.
Position Specific Responsibilities- Completes assessment of patient and family to determine Hospice needs.
- Provides a complete physical assessment and history and delivers professional nursing care using the nursing process.
- Writes and initiates Plan of Care in collaboration with the IDG.
- Regularly reevaluates patient/family/caregiver needs and revises the Plan of Care as necessary.
- Initiates the Plan of Care and makes revisions as patient status and needs change.
- Develops a care plan in conjunction with the patient/family/caregiver and IDG, establishing goals based on nursing diagnosis and incorporating palliative nursing actions.
- May administer medications and treatments as prescribed by the IDG-approved Plan of Care.
- Participates in on-call duties as defined by on-call procedures.
- Maintains productivity standards as set by organizational guidelines.
- Completes, maintains, and submits accurate and relevant clinical notes regarding patient condition and care given.
- Supervises ancillary personnel and delegates responsibilities as required.
- Participates in orientation, education, and development of team members.
- Maintains and upgrades professional knowledge and practice skills through continuing education and in-service classes.
Required: Associate or Bachelor’s degree from an accredited School of Nursing.
Preferred: 1 year experience practicing as a registered nurse. Experience as a Hospice CNA or Hospice LPN will be considered.
Certifications, Licenses, and RegistrationsRequired: Current, unencumbered (State of Employment) License as a Registered Nurse.
Required – Within 30 days of hire: National Provider BLS - American Heart Association (Vendor Managed).
Physical Demands And Work EnvironmentThe demands of this role require the ability to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are essential, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools is essential. This description provides a general overview and may vary by role and department.
