SummaryUnder general direction, collects, analyzes data, identifies expected outcomes of each patient, develops a plan of care with interventions, and continuously evaluates progress. Works collaboratively with the specialty health care team and other disciplines.
ResponsibilitiesUsing critical thinking and problem solving skills in a dynamic, systematic collaborative approach to providing and coordinating health care service to a defined population. Employs advanced nursing specialty knowledge to assess, plan, implement, evaluate, and interact with the patient.
Evaluates options and services to meet the individual’s health needs, promoting quality, cost‑effective outcomes. Develops and maintains patient tracking and reporting programs to identify gaps in the care delivery system.
Contributes to the mission and vision of ANTHC by providing necessary coordination of care for a specified population. Participates in unit‑based councils and process improvement committees.
Independently uses nursing knowledge to collect health data, analyze it to determine diagnoses, identify expected outcomes, develop a plan of care, implement interventions, and evaluate progress toward outcomes.
Engages in systematic evaluation of the quality and effectiveness of nursing practice, spearheads quality of care activities, and self‑evaluates practice against professional standards.
Adheres to Standards of Practice for Case Management, the ANA Nurses Code of Ethics, and specialty standards of practice. Contributes to professional development of peers and acts as a consultant on specialty issues.
Decides and acts on behalf of patients in an ethical, nonjudgmental, and nondiscriminatory manner, preserving patient autonomy, dignity, and rights. Collaborates with patients, families, and providers to form overall goals and a plan of care.
Considers safety, effectiveness, evidence based‑practice, and cost in planning and delivery of care. Assists patients and families in securing appropriate services, coordinating communication with insurance payers and external facilities.
