Position Summary:

The Nurse Practitioner – PPV is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy. Using the 6 Pillars of Success as the foundation, the NP upholds the Code of Ethical Conduct and fosters positive working relationships within the company, across departments, and with external stakeholders.

Position Summary

The NP provides assessments, disease and treatment education, and symptom management to patients facing chronic, complex, or life‑threatening conditions in palliative and hospice care settings. Collaborating with PCPs, specialists, and the assigned Compassus Medical Director, the NP ensures compliance with practice standards and codes. Flexibility to meet agency needs, including after‑hours emergency visits and admissions, is required.

Position Specific Responsibilities
  • Perform routine and emergency assessments for hospice care:
    • Complete recertification visits for patients in their 3rd benefit period and beyond, including admissions and transfers.
    • Document observations, treatments, and pertinent information.
    • Communicate findings with Medical Director, Attending Physician, DCS, and IDT.
    • Provide physical and emotional support during crises.
  • Prevent unwarranted hospitalizations by meeting patient and family needs  palliative care clinics, additional responsibilities include:
    • Conduct routine and emergency outpatient assessments.
    • Perform bedside evaluations, assist with diagnosis, treatment, and management of acute and chronic conditions.
    • Order and interpret laboratory and radiology tests within scope.
    • Prescribe medications, including controlled substances, as delegated.
    • Order durable medical equipment and other treatments.
    • Collaborate with palliative care physicians; inform primary physicians of services provided.
    • Coordinate referrals and maintain accurate patient records.
    • Assist families with treatment goals, advanced care planning, and training staff.
  • Communicate with Medical Director, attending physician, DCS, and IDT to address clinical and psychosocial needs:
  • Provide education to patients and caregivers about care, disease processes, symptom control, wound care, and prognosis.
  • Perform additional duties as assigned.
  • For hospice face‑to‑face visits, repeat routine and emergency assessments as above.
  • For palliative care, ensure a minimum of 4 days of availability per month, triage consults by acuity, and perform bedside education.
  • Supervise nurse trainees and participate in supervision of medical, resident, fellow, and allied health trainees.
Education And/or Experience
  • Master’s degree in Nursing required.
  • For hospice care:
    Minimum 1 year nursing practice or equivalent experience; hospice, oncology, homes‑health, or long‑term care preferred.
  • For palliative care:
    Minimum 5 years nursing experience; preference for oncology, hospice, geriatrics, or intensive care backgrounds.
Skills
  • Mathematical:
    Add, subtract, multiply, divide in whole numbers, fractions, and decimals; compute rates, ratios, and percentages.
  • Language:
    Read, analyze, and interpret professional journals and regulations; write reports and procedures; communicate effectively.
  • Other:
    Strong written and verbal communication, organization, time management, flexibility for nights and weekends, travel as needed, confidentiality of patient information.
Certifications, Licenses, and Registrations
  • Active, unencumbered CNS or Nurse Practitioner license in the state of employment.
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric.
  • Active NPI number.
  • DEA certification if state allows prescription of controlled substances by APPs.
  • Board certification in Hospice and Palliative Care preferred.
State Specific Requirements
  • All states except Iowa:
    Current collaborative physician agreement in place.
