ResponsibilitiesThe nurse navigator will support the navigation of patients through their hospitalization for optimization of care and outcomes. Will assist with scheduling follow up appointments in order to improve continuity of care and streamline the intervention process. To collaborate with other members of the health care delivery team including physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services.
QualificationsProspective candidate will possess current Alabama RN Licensure and current BLS Certification. ACLS and PALS certification preferred. Must promote customer service initiatives.
