Care Coordinator / Life Skills Instructor – Youth & Family HCBS

Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services, and programs addressing the social determinants of health such as housing and employment. Our health centers serve five Central Indiana counties, providing services to all ages and walks of life.

Now interviewing: Care Coordinator – Life Skills Instructor – Youth & Family Home & Community-Based Services – Anderson. $1,000 Hiring Bonus.

The Care Coordinator/Life Skills Instructor provides behavioral health and/or child welfare related services to clients and their families in community and home settings in order to achieve their identified goals.

• Bachelor's degree in social work, psychology, counseling, or a directly-related human service field from a college/university accredited by the US Department of Education.
• Two (2) years directly related experience in past professional and/or nonprofessional roles.
• Valid driver’s license, current automobile insurance, and a driving record that meets the organization’s guidelines.
• Ability to use a private car to transport self and others.

Drug screen, TB test, and extensive background checks (including criminal history, sex offender registry search, state central registry check, education verification, and professional references) are required of all Aspire employees.
