A leading critical illness recovery hospital is seeking a Case Manager to develop patient-specific discharge plans and coordinate care in a fast-paced hospital environment. The ideal candidate will have current licensure as a clinical professional (RN, RT, or SW) and experience in discharge planning. This role requires building relationships with payor sources and participating in multidisciplinary team meetings. The position offers flexible shift scheduling, with minimum availability of 2 shifts per month during weekdays.
