Join Trinity Health as a Community Health Worker to help patients and families navigate community services and promote healthy behaviors. The position is funded by a grant from the Robert Wood Johnson Foundation and includes screening for basic human needs.

Roles & Responsibilities
  • Establish trusting relationships with patients and families while providing support and encouragement.
  • Conduct intake interviews, enroll patients, and refer them to appropriate community resource programs.
  • Provide ongoing follow‑up through phone calls, home visits, and other settings.
  • Assist patients with applications, registration forms, and eligibility determination.
  • Help patients set personal goals, attend appointments, and use transportation resources.
  • Act as patient advocate and liaison with community agencies (schools, human services, health care for the homeless, hospitals, support groups).
  • Document care in Data Shop and other software within 24 hours of contact.
  • Attend staff meetings, trainings, and manage assigned caseload of patients.
  • Maintain communication with care management staff and report progress clearly.
  • Follow up continuously from initial identification through closure, ensuring coordinated care.
Minimum Knowledge, Skills, and Abilities
  • High School Diploma or GED. Completion of a Community Health Worker formal training program or relevant experience preferred.
  • Experience in a multi‑cultural and community‑based setting (1‑2 years preferred).
  • Basic medical terminology knowledge preferred.
  • Basic computer skills.
  • Strong communication: listening well and using language appropriately.
  • Ability to build positive relationships with public health staff and community organizations.
  • Understanding of the community served and community connectedness.
  • Empathy, encouragement, and motivation to support patients emotionally.
