Outpatient RN: Care Coordination & Triage

Trinity Health IHA Medical Group in Ann Arbor, MI, is seeking a compassionate Registered Nurse to join our outpatient care team. The RN will perform clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience.

You will collaborate with providers, care managers, and other healthcare partners, manage medication refills and test results, and support patients through transitions of care and post-discharge follow-up.
