Cofr Nurse Care Manager

A COFR (County of Financial Responsibility) Nurse Care Manager, under the general direction of the Director of Health Information Services, provides and/or arranges comprehensive, person-centered services for Health West consumers who are placed and/or residing outside their county of residence.

This role combines care management, coordination, and oversight of services with clinical nursing responsibilities, ensuring medical, behavioral health, and community supports are effectively integrated. Responsibilities include assessment, treatment planning, care coordination, monitoring, advocacy, and direct nursing functions to support high-quality, compliant, and outcome-driven care.

• 1. Possess a valid State of Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
• 2. Possess a valid driver’s license and the ability to obtain a Michigan Driver License within 30 days of employment.
• 3. Must have reliable transportation that may have to be used to carry out job duties of this classification.

• 2. Lived experiences with mental illness/developmental disabilities/substance use disorders valued.
• 3. Individuals in Recovery and individuals with experience in Armed Services valued.

An employee in this class performs generally sedentary work requiring the occasional lifting of objects weighing thirty-five (35) pounds or less.

An employee in this class generally works from home, although travel to County locations and out of the County travel will be required.

Appointees must satisfactorily complete a probationary period before the appointment will be considered permanent.

The purpose of this recruitment is to establish an eligible list from which to fill present and future vacancies in this classification in the County system. For each opening, the Department Head has her/his choice of the candidates who meet the required minimum entrance qualifications. This list remains in effect for three months unless exhausted or extended.
