PRESERVATION CASE MANAGER - MiFamily Together
Full Time 40 hours per week.
We are searching for Preservation Workers to join our Samaritas Family Preservation Team! This position will be in the MiFamily Together (MiFT) program, covering Washtenaw, Lenawee, and Monroe counties.
MiFamily Together is a family preservation and reunification, home‑based program that offers families flexible services that adapt to their needs. It is designed to decrease the risk of child maltreatment while also helping to increase the wellbeing and prosperity of each family member. MiFT workers will provide services to families, in their homes, for up to six months, utilizing Protective Factors and the 2
Gen Approach. They will meet with families every week, complete comprehensive assessments, collaboratively develop family goals, and assist families in attaining new skills and resources.
Annual salary range: $43,888.00 to $47,840.00. Varied shifts may include evenings and weekends.
Duties and Responsibilities- Provide in‑home crisis‑oriented treatment, education, and support to family units where one or more family members may be placed outside the home or be at risk of removal.
- Provide a range of interventions to children, adults, and families, using acceptable treatment methodologies aligned with the Family Preservation best‑practice model.
- Actively participate in team meetings, keep all members informed of case status and progress, provide oral and written reports as needed, and make recommendations about case goals, alternative service plans, action steps, and activities for treatment plans.
- Assist clients to clearly define presenting problems, devise methods and goals for resolution, assess their progress toward treatment goals, and plan termination.
- Present cases to supervisor for review of treatment plans and progress on a regularly scheduled basis.
- Provide appropriate referrals for clients requiring additional or alternative services.
- Assume therapeutic responsibility for all cases on assigned caseload; conduct intake, assessment, and scheduling of all assigned clients.
- Collaborate with referral sources, agencies, institutions, and individuals who have important roles in the family's life situation. Provide advocacy and professional consultation work with referring organizations, schools, courts, and other social‑service agencies as needed.
- Complete client follow‑up at intervals as outlined.
- Complete and maintain all reports, statistics, and assignments as required. Enter all required data in required systems in a timely manner.
- Participate in all required training and effectively implement learned skills.
Licensure/Certification
- Minimum of a bachelor’s degree in a human services field or education; or two years' experience working with children and/or families and an associate degree or equivalent in human services or education, with active pursuit of a bachelor’s degree considered.
- Field experience working with multi‑problem children and families; ability to relate to and engage with these families preferred.
- Experience in strength‑based, family‑centered, solution‑focused approaches beneficial.
- Participation in a system of 24‑hour availability to families as required; weekend and evening hours required.
- Valid driver's license with good driving record; reliable ongoing personal transportation suitable for safely transporting clients; current automobile registration and proof of automobile insurance.
