Home Health Care, Inc. is seeking an RN Case Manager to coordinate care for patients in the Northwest Twin Cities and St. Cloud corridor, managing caseloads from initial assessment to discharge.
You will perform OASIS assessments, develop plans of care, collaborate with PT/OT/SLP and physicians, and use Homecare Homebase for documentation. This full-time role offers a Monday–Friday schedule with occasional weekends.
