Position open to all qualified candidates.
Location: 3 Harry S. Truman Parkway, Annapolis, MD 21401
Hours: 40 hours per week

Job DescriptionNature of Work:
Receive and interpret incoming client referrals to assist Medicaid-eligible/pending recipients in accessing and appropriately utilizing their health care benefits. Verify Medical Assistance coverage and enter client data into electronic files.
Provide Medical Assistance/Health Choice education, navigation, access to care and community resources to eligible/pending recipients via phone, email and/or home visits. Provide short-term care coordination as needed. Provide quarterly and as-needed outreach activities to Medical Assistance participating providers, health centers, and hospitals to educate, inform, and update them on Medical Assistance, Health Choice, Administrative Care Coordination (ACC)/Ombudsman services, and community issues and resources.
Attend biweekly staff meetings and actively contribute to the planning, implementing, evaluating and revising of procedures and practices to improve the overall efficiency and functioning of the Administrative Care Coordination program.
Submit timely monthly reports and statistics. Assist with maintaining and filing client records, including preparing records for eventual discharge according to state and local guidelines.
Minimum QualificationsEducation:
High School Diploma or Equivalency (GED).

Preferred Qualifications:
Knowledge of Medicaid and care coordination services.
