Gentiva seeks a dedicated Hospice Registered Nurse Case Manager to deliver compassionate, patient‑centered care in patients’ homes within Calhoun, Clay, Cleburne and Talladega Counties. You will assess needs, develop plans of care, and coordinate with an interdisciplinary team to support patients and families facing life‑limiting illness.
The role requires an active RN license, 1+ year RN experience (hospice/palliative care preferred), CPR and a valid driver’s license.
