Home Health:
Start of Care/Admission Registered Nurse (RN)
Primarily responsible to plan, organize, direct home care services. Experience in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Start of Care/Admission Registered Nurse (RN) for Home Health
Wage range: $88,000-$90,000 annually, depending on experience
Service Area:
Twin Cities Metro Area
. Plus, mileage reimbursement of .70 cents per mile.
Schedule:
Full-Time. 1.0 FTE /
Monday – Friday, 8:00am-5:00pm (with 2 holidays per year rotation and NO WEEKENDS!)

Position Functions & Responsibilities:
- Frame the discussion and builds understanding with clients that Lifespark starts with the clients’ immediate needs and wants (short term) while also working to build a longer-term plan to help the clients avoid the roller coaster of health care crisis™ thereby protecting and improving their independence and quality of life.
- Effectively articulate and differentiate the Lifespark Life Care Management model.
- Provide assurance and relief (peace of mind) to clients and families that Lifespark will effectively address their needs and wants.
- Make the clients feel comfortable and well‑cared for.
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Initiates the plan of care
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development.
- Completes necessary paperwork accurate at admission and submits to medical records the same day.
- Completes emergency preparedness plan with client/caregiver at admission visit
- Communicates with IDT post admission or non‑admission including client status and a summary of the plan of care for client.
- Ensure positive/professional communication on team
- Customer service resolution; elevate concerns
- Prepares clinical notes and updates the primary physician
- Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders
- Communicates with community health related persons to coordinate the care plan
- Honors Client Confidentiality, Rights, Privacy, and Reporting Maltreatment Expectations
- Understands and Accepts Clients’ Diagnosis
- Understands and accepts emotional needs of client
- Accepts unique client symptoms and behaviors
- Follows all emergency preparedness and response procedures
- Identifies home safety issues and cascades info to the IDT
