Per Diem Transitional Counselor - Youth Adult Services (YAS)
BHcare is seeking compassionate and reliable Per Diem Transitional Counselors to join our Young Adult Services (YAS) Program, supporting young adults on their journey toward independence and community integration. This role is an excellent fit for those passionate about mental health, recovery, and empowering young people to reach their potential.
Base pay range20.00/hr - 20.00/hr
Responsibilities- Provide hands‑on support with daily living skills such as personal care, meal planning, cooking, cleaning, money management, and community integration.
- Assist young adults in developing and implementing individualized recovery plans, coordinating with treatment teams, natural supports, and community resources.
- Deliver counseling, emotional support, and skill‑building focused on independence, self‑esteem, and quality of life.
- Support residents through crisis intervention and ensure safety and well‑being at all times, including overnight shifts.
- Maintain accurate documentation, progress notes, and database entries.
- Provide transportation and accompany residents to appointments, YAS events, and activities as needed.
- Collaborate with clinical teams, nursing services, and outside providers to ensure comprehensive care.
- Stay engaged, awake, and alert during all shifts, ensuring a safe and supportive environment.
- Compassionate, patient, and reliable individuals committed to empowering young adults.
- Strong communication, problem‑solving, and teamwork skills.
- Understanding of (or willingness to learn) Motivational Interviewing, trauma‑informed practices, and integrated treatment for co‑occurring disorders.
- Experience in residential, mental health, or human services preferred (training provided).
- Valid driver’s license and reliable transportation required.

Minimum Qualifications- Two years of college education (human services or behavioral science preferred) or high school diploma or equivalent plus two years direct care work experience in behavioral health residential care.
- Must have and maintain a valid driver’s license, an acceptable driving record, and be insurable under the organization’s auto insurance plan.
- Must be proficient in Microsoft 365 and able to navigate through an electronic health records system (EHR).
- Basic Life Support (BLS) certification (ability to obtain and maintain).
