Opportunity:

PT - Physical Therapist

Job Details:

• 13-week contract with ASAP start date

• 37 guaranteed hours per week with flexibility up to 40 hours

• Monday through Friday schedule from 8:00 AM to 5:30 PM

• Occasional travel to other branches as needed with mileage reimbursed at government approved rate

• Can be placed on low census call with 2-hour minimum call back pay

• Rotate one Saturday every 6-8 weeks with compensatory day off during the week

• No holiday requirements

• Required certifications: AHA BLS, Wisconsin state licensure, NPI number required

• Bachelor's degree required, Master's degree in Physical Therapy preferred

• Home health experience required with adult/geriatric population

• Experience with orthopedic, neurological, and functional mobility diagnoses

• Laptop provided for point of service documentation using Net Smart EMR

• Cell phone provided for GPS navigation and communication

• Productivity expectation of average 4.5 visits per day

• Evaluate, treat, and plan patient care according to Physical Therapy standards

• Document to meet regulatory and reimbursement requirements

• Contract eligible for 36 hours of unbillable low census hours

• During flu season, unvaccinated staff must wear masks at all times per facility policy
