Key Responsibilities:
• Conduct thorough client assessments
• Collaborate with healthcare teams for primary care
• Administer medications as required
• Provide wound care and chronic disease management
• Implement rehabilitative and palliative care programs
Requirements:
• Must be a licensed Practical Nurse in the province
• Community health experience preferred
• Valid driver's license and reliable vehicle required
• Satisfactory Vulnerable Sector Search mandatory
• Ability to wear Personal Protective Equipment (PPE)
