Base Pay Range

$25.00/hr - $28.00/hr

The Case Manager (CM), reporting to the Case Management Supervisor will work directly with clients and their family/support system of various diverse socio-economic backgrounds to establish rapport while facilitating wellness around complex physical and/or mental health conditions to improve quality of life. The CM works as a member of the CiA team providing health and social services to frail elders and adults with disabilities to meet CiA’s mission to create opportunities where people can learn, grow and age independently with dignity and community.

Duties
  • Work with older adults in Contra Costa County to help improve their quality of life by developing care plans, facilitating access to resources, advocating for client rights, and promoting self-sufficiency and empowerment.
  • Build rapport with clients and families with diverse socio-economic backgrounds.
  • Conduct prescreening, home visits, assessments, phone calls and outreach linkages for clients; meeting program guidelines.
  • Build relationships with possible linkage/referral sources around healthcare, social services, mental health, housing, employment, and education.
  • Coordinate all necessary referrals/linkages on behalf of client needs.
  • Comply with paperwork requirements, including maintaining accurate, current and concise records, such as referral/intake screens, logs, progress notes, follow-up monitoring, demographic and other statistics, and meeting all program deadlines.
  • Submit required reports to Supervisor in a timely manner, meeting all program deadlines.
  • li>Any other duties required by the Supervisor.
Minimum Qualifications
  • High school diploma or equivalent, or associates degree in related field such as social work or psychology.
  • Bachelor’s level degree preferred, or equivalent experience in case management with older and/or disabled adults; nonprofit experience preferred.
  • Working knowledge of MS office, Google suite, and basic internet usage.
  • Experience working with the Older Adult population preferred.
  • Bi-lingual English/Spanish or English/Tagalog desired.

Program History Adult Day Health Care (ADHC) is a program that was created in California in 1978 in response to a need to have an alternate solution to institutionalization. The program mirrors the skilled nursing staffing model, but is based in the community and typically operates during the day on weekdays. It is akin to a senior center for those who can no longer attend a senior center independently.

It is especially beneficial for those who need additional support to do things like ambulate, eat, toilet, participate in activities and more. Staff provides therapeutic activities disguised as fun throughout the day, making socializing and exercising meaningful and pleasurable. This makes folks want to come back each day!
