Job SummaryCore to the Arch Well Health approach is to have a robust clinical care team focused on the needs of our members. Care teams deliver excellent and compassionate primary care and coordinate care with the broader healthcare delivery system. The Licensed Practical Nurse (LPN) is a critical role supporting the PCP day-to-day, ensuring efficiency. The LPN will work closely with teammates to greet and room members, assist in assessing health conditions, and perform routine diagnostic testing during appointments.
Duties and Responsibilities- Conduct intake with patients, measuring and recording vital signs, patient interview, and medical history; perform examinations and routine screenings, ensuring patients are informed and all questions are addressed.
- Listen and engage with patients throughout the appointment, reporting changes to PCP, specialists, and other clinical personnel.
- Process patient phone messages, return calls, route calls, and call patients to obtain and relay information.
- Perform venipunctures and IV starts (as allowed by State).
- Change dressings, apply bandages, remove sutures, and perform first aid; use CPR as required under physician supervision.
- Ensure compliance with OSHA safety standards and maintain accurate legal and ethical documentation.
- Provide health coaching to a defined group of patients to support healthy lifestyle choices; follow up with coached patients via weekly calls under physician supervision.

- High school diploma or equivalent (GED) required.
- Graduation from a nationally accredited Licensed Practical Nurse program.
- Current, active LPN license in the state of employment.
- Minimum 1 year of LPN experience, preferably in a clinic setting.
- BLS for Healthcare Providers required.
- Experience with geriatric patients is a plus.
- eClinical
Works experience preferred, or similar EHR.
