Are you looking for a challenging and rewarding position as a Hospice RN? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family for our Gilbert AZ location servicing the East Valley and surrounding areas!

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits that allow for clinical observation of the performance of assigned team members, participate and provide feedback at regular interdisciplinary group meetings, and help develop, review, and revise the patient/family plan of care.

• Current Arizona License in good standing
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient’s status and needs on a full‑time on‑call basis

• Initial and ongoing assessment of patient’s physical, functional, psychosocial, and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with Advisa Care Hospice Policies and Procedures and Medicare compliance
• Consult and educate the patient/family on disease process, self‑care techniques, end‑of‑life care, and ethical concerns
• Supervise paraprofessionals providing services to patient
• Prepare clinical and progress notes, participate in IDG conferences
• Participate in Hospice performance improvement program
• Maintain a high level of customer satisfaction with our clients, families, and caregivers
• Communicate information using current processes and technology within the organization
