Agathos hulpverlening zoekt een intensief ambulant jeugdhulpverlener om kinderen en jongeren in complexe situaties te ondersteunen, meestal bij het gezin thuis. Je werkt nauw samen met gemeenten en hulpverleningsinstanties om de zorgvraag af te stemmen en de kracht van het gezin te versterken.
Je werkt zelfstandig maar bent ook een echte teamspeler, met aandacht voor dossierregistratie, kwaliteitsverbetering en afstemming met het jonge netwerk.
