RN/LMSW Case Manager, Inpatient Rehabilitation

Position at Lifepoint Health® in partnership with Peak Rehabilitation Hospital, a joint‑venture between Duke Health, Wake Med, and Lifepoint Rehabilitation. This full‑time role focuses on delivering high‑quality inpatient rehabilitation services.

• Completes departmental orientation, initial and annual competencies.
• Assists with departmental performance‑improvement initiatives, collecting and reporting data as requested by the supervisor.
• Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care or discharge, and develop solutions.
• Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedules family conferences and/or communicates with caregivers following each team conference, and more often as needed to keep patient and caregiver informed of progress and discharge plans.

• Current Registered Nurse or Social Work licensure or licensure as a Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, or RN‑BC).
• Minimum of 2 years social work or case management experience in an inpatient setting; acute/rehabilitation hospital experience preferred.
• Excellent organizational, time‑management and analytical skills.
• Strong teamwork orientation and cooperative approach.
