Position Summary:

Under the direction of a Registered Nurse, the Licensed Vocational/Practical Nurse provides skilled nursing care for assigned patients. Responsibilities include performing medical treatments, providing patient education, and participating in care planning.

Qualifications:

• Graduate of an accredited vocational/practical nursing program.
• Current license within the State or valid compact.
• Current BLS certification.
• 1+ years of experience in hospice, palliative, or home health preferred.

Core competencies include skilled nursing performance, communication skills, ability to work independently, and compassionate care for patients and families.

Functions & Responsibilities include monitoring patient conditions, following care plans, educating patients and families, documenting care, and participating in quality improvement activities.
