RN Admission Specialist – Home Health

• Solo Visits: Deliver direct care in the field with freedom and purpose
• Purpose-Driven Pay: Earn $200 per admission visit
• Client-Centered: Build trust as the first clinician in the care journey

• Perform comprehensive patient assessments and OASIS documentation
• Initiate individualized care plans
• Recommend and coordinate additional services
• Communicate with providers and caregivers to ensure safe transitions

• Minnesota RN license in good standing
• Admission and OASIS experience
• Strong communication and time management
• Reliable transportation
