Home Health Scheduler

Overview

Supports the mission, vision, and values of the organization. The Home Health Scheduler will support and manage the PRN Home Health & Therapy business efficiently. This position supports the organization by overseeing scheduling and the coordination of patient care. Their focus is to oversee scheduling of patient visits, ensure timely and accurate intake processes.

• Schedule patient visits with clinicians
• Coordinate with PRN Home Health & Therapy, LLLC clinical staff to assure efficient admission of new referrals.
• Answer telephone calls using professional telephone etiquette while maintaining patient confidentiality
• Ensure effective and timely coordinator of patient home health services through the completion of required documentation and computer data entry for new referrals, as well as timely transfer of pertinent medical data to client’s physician and therapists.
• Assist as needed in calls to patients, caregivers, providers, facilities to coordinate and oversee patient care

• Prefer a minimum of one year of clinical experience in healthcare settings
• Excellent verbal and written communication skills
• Ability to work individually or in a team environment; self-motivated with minimal supervision
• Good judgement and discretion when dealing with sensitive topics
• Demonstrates tact, patience and good personal hygiene
• Proficiency with Microsoft Office Suite (Word, Excel, Outlook)
