We are looking for a licensed Physical Therapist to join our compassionate and growing team in the Fond du Lac area. This is a great opportunity to build meaningful connections with patients while enjoying autonomy and flexibility in your schedule. As a clinician with PRN HH&T, you’ll primarily work in a home health setting, with additional opportunities for cross‑training and work in skilled nursing facilities if desired.

PRN Home Health & Therapy is a family‑owned, Wisconsin‑based company headquartered in Appleton and serving 20 counties across central and eastern Wisconsin. We provide skilled nursing, physical therapy, occupational therapy, and speech therapy services to patients in the comfort of their homes.

• Perform in‑home patient evaluations and develop individualized treatment plans
• Deliver evidence‑based therapy to improve patient mobility, function, and quality of life
• Collaborate with patients, families, and clinical team members to support care goals
• Accurately document treatments and progress, including discharge summaries
• Supervise and guide Physical Therapist Assistants (PTAs) in treatment plans
• Educate patients and caregivers on safe mobility practices and discharge planning

• Current Physical Therapist (PT) license in the state of Wisconsin
• Valid driver’s license and reliable transportation
• Home health experience preferred but not required—we provide training!
