Home Health RN Registered Nurse (PRN) – Empath Home Health

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health RN to provide skilled nursing care for patients in their homes in Arcadia, FL and surrounding areas. In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

Locations: Home health visits in Arcadia, FL and surrounding areas

Schedule: PRN role offering 5-10+ visits per week. Required weekend/holiday on-call rotations. Full-time may be available if desired!

With more than 60 years of expertise, Empath’s Home Health service line provides skilled nursing, therapy, and personal care services that help people recover and maintain independence t of Empath Health’s not-for-profit network, the program serves patients across 16 Florida counties and is accredited by the Joint Commission and the Accreditation Commission for Health Care.

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
