Description
Under the direction of the Medical Director and Cancer Care Program Director, the Patient Navigator is responsible for ensuring that all new cancer patients when identified are contacted and followed through the screening/diagnostic evaluation process; in addition, the Patient Navigator assists patients with general navigation through the healthcare system to include patient education and support needs; provides needed information for the Cancer Conference when cases are presented;
provides general follow-up for patients as needed during the treatment phases.

Required Qualifications:

California Registered Nurse License - Upon hire:
National Provider BLS - American Heart Association - Within 1 year of hire:
Oncology Certified Nurse (OCN) or Advanced Oncology Certified Nurse Specialist (AOCNS) - 3 years experience in Oncology Nursing
Preferred Qualifications:
- Bachelor's Degree in Nursing
- 5 years experience as a Registered Nurse in a healthcare setting, with 3 years in Oncology Nursing

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
