Field Onboarding Specialist I (Maricopa County, AZ / Field-based)

Join Freedom Care as a Field Onboarding Specialist. You will visit patients and caregivers in their homes, acquire signatures for necessary documents via a Surface Pro and iPhone, provide a brief orientation, and participate in statewide semi‑weekly meetings with colleagues in Phoenix, AZ.

• Visit patients and caregivers in their homes
• Acquire signatures for necessary documents via a Surface Pro and iPhone
• Provide a brief orientation to ensure caregivers and patients are set up for their care plan
• Participate in state‑wide semi‑weekly meeting with colleagues in Phoenix, AZ
• Serve as backup caregiver in case primary or secondary caregivers are unavailable, ensuring ADL/IADL care needs are met
• Conduct routine supervisory visits to patients’ homes to review that appropriate care is being administered

• 1+ year experience in a field‑based role
• Direct Care Worker (DCW) certification or willingness to obtain certification
• Excellent written and verbal communication skills
• Reliable transportation and ability to travel as required
• Self‑starter personality with ability to work independently with minimal supervision
• Ability to work remotely as required
• Bilingual in Spanish (preferred)
