We provide support to our therapy teams with a robust clinical support team: a field preceptor, an IT specialist, scheduler, therapy supervisor, and nurse manager. Having a strong clinical support structure is important to supporting our field clinicians in managing their caseload appropriately while not feeling alone.
As a Banner Home Health Physical Therapist, you will enjoy working in sub‑assigned geographic territories with smaller travel distances than many other agencies. You will enjoy an autonomous but supportive work environment and have the option to work pay per visit or hourly. We offer mileage reimbursement and will provide you a company cell phone and a tablet for documentation in our electronic medical record system (Home Care Home Base).

This position will provide visits in Casa Grande, Maricopa, Eloy and Arizona City.
Banner Staffing Services (BSS) also offers Registry/Per Diem opportunities within Banner Health. Registry/Per Diem positions are utilized as needed within our facilities. BSS Registry positions do not have guaranteed hours and no medical benefits package is offered. BSS requires completion of post‑offer Occupational Health physical assessment, drug screen and background check (includes employment, criminal and education).
Position SummaryThis position is responsible for the evaluation, planning, administration and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.
Core Functions- Evaluates patient's functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals. Completes comprehensive evaluation within the established time frame set by the department. Identifies and prioritizes key limiting factors. Establishes treatment goals that are functional, measurable, patient related and reflect the key limiting factors. Collaborates with patient and family/caregiver when setting goals.…
