LPN-LTC-Sedona-AZ

Delivers nursing care to residents in a long-term care setting under the direction of a Registered Nurse or physician. Administers medications and treatments, monitors resident status and supervises Certified Nursing Assistants. Maintains accurate clinical documentation and communicates changes in condition to the care team.

• Graduate of an accredited practical or vocational nursing program:
  Required
• Long-term care or skilled nursing experience:
  Preferred

• Licensed Practical Nurse or Licensed Vocational Nurse licensure in the state of practice:
  Required
• Basic Life Support (BLS) certification:
  Required

• Administer medications and treatments per physician orders and facility policy.
• Perform resident assessments and monitor for changes in physical, cognitive or behavioral status.
• Supervise and delegate appropriately to Certified Nursing Assistants.
• Complete accurate and timely documentation in the resident record.
• Communicate changes in condition to the Registered Nurse, physician and family as appropriate.
• Follow infection control, safety and regulatory requirements for long-term care.
