Home Health Licensed Practical Nurse (LPN)

Become part of our caring community and help patients regain independence in their homes. Working closely with a dedicated team of physicians and clinicians, you’ll provide personalized one-on-one care that supports recovery.

• Base pay: $44,600 - $61,400 per year
• Per visit/unit: $28.00 - $40.00

• Provide professional nursing services under RN direction, compliant with state laws, Center Well policies.
• Administer skilled nursing interventions, using a holistic, therapeutic approach and teaching activities.
• Apply clinical standards to facilitate problem resolution and achieve individualized patient goals.
• Consult RN Supervisor for changes to plan of treatment; accept verbal orders from physicians as permitted.
• Implement goal/outcome driven nursing plan of care.
• Maintain timely communication with patients, physicians, managers, and team members regarding patient/family needs and status changes.
• Maintain professional conduct regarding confidential information, sharing only when necessary and in the best interest of the patient.
• Ensure supplies, materials, and equipment are available for safe and effective plan of care.
• Prepare, submit, and maintain documentation, including visit notes.

• Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, reliable transportation.
• Current CPR certification.
• Minimum two years experience as LPN/LVN in a clinical setting, preferably home health or hospice.
